Mr. Bungle Mr Bungle Clown Face
After some research I've discovered this Green Clown Face with Red Guts and only one sleeve design was sold on the Disco Volate Australian tour back in 1996. There are long sleeve variants as well, one with multiple shields down the one sleeve and another with just a single shield like the short sleeve.
