By far the best exodus shirt i ve seen even though smoke looks like flowers:)))))))
Woman of Merch ...
I was wondering if the "smoke" you called flowerlike is rather supposed to be the tissue of the uterus walls and the egglike thing is ment to be the bursted amniotic sac (with vessels in it's membranes)?! In that case this might be an original "uncensored" artwork, that might have been considered too "disgusting" back in the days regarding prudery in America. I always thought the light blue background looked silly! The LP would have looked much better with a cover like the print of the shirt! If I'm wrong with my assumption then the one who composed the artwork for the shirt was feeling the same about the babyblue background and showed extraordinary creativity in correcting what he disliked and rethinking the siamese twins environment.

Here is the tag, definitely a French print from the mid 80’s. My memories were right: it was indeed for sale in Enfer Magazine as muscle shirt and sweatshirt too. So definitely a non official painting designed by French company but indeed the artist did a killer job, replacing the blue surgical background for maybe a failed attempt of separating the two evil eternal twins (©️Depressive Age ahahahah) by the « tissue of the uterus walls and the egglike thing is meant to be the bursted amniotic sac (with vessels in its membranes) » according to your words Sister. So definitely not an unused or censored official painting for the album cover. And definitely not a real bootleg cause these French shirts printing company had some sort of agreement with the labels and bands according to Mathew aka Fuggle74 who did his research concerning the French Maiden shirts which had almost all a two lines separated words for the band name :
IRON
MAIDEN

What is that last picture? LOL

That's two fotos combined to one of the uterus tissue (above) and the amniotic sac (below) to give proof to my assumption in the text.
