According to Sammy Duet on a podcast called Requiem Metal, the band members were all in disagreement about what the album cover should be. The owner of Rotten Records, Ron Peterson, was really pushing for the album to comeout and he had a friend who was really big into serial killer memorabilia and had one of Gacy's deathrow paintings. Ron showed either a picture or the Gacy painting itself to the band and they weren't fans of the actual painting but they really liked the unsettling meaning behind the painting. Rest is history
