My take on this is that it's one of the many patches that were issued for Reading festival 1980. At this precise occasion, Ozzy's band was (mis)billed as "Ossie Osbourne's Blizzard Of Oz". As it happens, the poster of the festival used the exact same font as the one of this patch.
It leads me to think that this might have been a somewhat official patch release (vs. the many bootlegs which were produced for this edition of the festival).
