Yeah, this is definitely one of the oldest Pyogenesis shirts which exist!
I’ve another information about it – as proof for the existence/official release of this shirt:
Tim of Pyogenesis has worn this shirt at the recording-sessions for the album “Sweet X-Rated Nothings”.
You can see that in the booklet of the "Sweet X-Rated Nothings" album.
