It comes from a private collection which is partly for sale. It's in a good to very good condition, but it's had a previous life. There are just slight discolorations on the right sleeve (back), and on the bottom back of shirt (see pictures). Otherwise no holes that I could see.
The collar tag reads XL.

Yup, having been issued in 1993 only, looks like this tee mashed up a portion of the Cause Of Death artwork and logo from The End Complete, released the year before.
