It's all too real. That dead person was Mayhem's vocalist at the time, ironically he went by the stage name "Dead." He slit his wrists, but since it was taking too long to bleed out he blew his brains out with a shotgun. His so called "friend" and fellow band member took pictures when he found his body, and then this one ended up on the "Dawn of the Black Hearts" album cover.
This band's story is very interesting, it involves church arson, murder, suicide, and a lot of fucked up stories. If anyone has earned the name 'Mayhem' it's this Norwegian band.

Both on Ebay several years ago. The Anvil patch I bought before their sudden rise in popularity with their movie.

No. Steve Rowe is a friend of mine and he sent it to me in the mail in exchange for an Ultimatum patch. Soundmass does have some though.
